what size hoses does this take and does it comes with hoses, clamps? does it include everything to just hook up to the pool?
also, is this a 21" sand filter and how much sand does it take??

Yes this comes with all the hoses and clamps needed to just hook up, the hoses are 1.5". This is a 21" sand filter which requires 250 Lbs. of sand.

When should I change the sand in my filter?

You should change the sand in your filter every 5-7 years. You can generally tell that it is time if your pool water is not as clear as it normally is or when you notice the interval between backwashes is shorter. You can use > this guide to walk you through it.
